Effect of Sotagliflozin: SOLOIST-WHF Trial
Sotagliflozin is an SGLT2 inhibitor, but also inhibits SGLT1, which primarily exists in the gut and appears to delay glucose absorption. The SOLOIST-WHF trial showed that sotagliflozin has salutary effects on CV outcomes among patients with T2DM and HF. Patient-centric outcomes such as days alive and out of the hospital were also improved with sotagliflozin compared with placebo.
Individualization of treatment for hypertensive patients with elevated heart rate
By the time patient treatment plans are approached positively, poor adherence/persistence shows 50% reduction in its effectiveness. SPC is recommended as 1st line treatment. Also among RAAS blockers, ACEi should be preferred over ARBs as 1st line of treatment. In patients with elevated BP and HR, SPC including BB+ACEi shows a rationale and effective treatment strategy.
The 2018 ESC/ESH and 2020 ISH core treatment Strategy: Importance of SPC
SPC medicines play a significant role in improving adherence and clinical outcomes of patients requiring multiple drugs for BP control. FDC medicines improve patient adherence to medication, improving BP control. Low dose SPCs are recommended as 1st line therapy by the latest guidelines for most patients, as it improves adherence, simplifies treatment, improves persistence with medications.
Pulmonary Artery Wedge Pressure Change with Passive Leg Raise
Patients were classified into two groups: PH from left heart disease and PAH. Compared to pulmonary arterial wedge pressure at rest, PLR increased PAWP in both groups. PLR causes an increase in PAWP in both LHD and PAH because of preload increase with improved venous return in patients with PH. But, the disproportionate increase in PAWP with PLR in LHD could help differentiate it from PAH.
Stable Ischemic Heart Disease Treated with an Invasive Vs. Conservative Strategy
A study was presented which aimed to examine the frequency and outcomes of anatomic complete revascularization and ischemic/ functional compared with incomplete revascularization in patients treated with an invasive approach. Anatomic and functional CR were achieved in 43.3% and 58.3% of patients resp. It was concluded that ACR was more strongly correlated with improved outcomes than FCR
Rhythm Control Versus Rate Control Trial
An examination of rhythm control with ablation decreases all-cause mortality and heart failure (HF) hospitalizations. Patients in the ablation-based rhythm control group showed numerically fewer primary outcome events, and higher improvement of left ventricular function, quality of life and reduction of N-terminal pro-B-type natriuretic peptide
Medicines that could possibly risk high blood pressure: peer-reviewed
Where as per a study, antidepressants, nonsteroidal anti-inflammatory drugs, steroids, and estrogens are some of the known drugs to raise blood pressure and is possibly standing in the way of around 2.2 million of Americans from having it under control, says a researcher at Beth Israel Deaconess Medical Centre in Boston.
